MSDE requires all students to have certain documentation in student files. The Emergency Contact Form and the Heath Inventory Form must be completed for all students enrolled at Christ Our King Preschool and be received by the director before the first day of school.

 

The Medication Administration Authorization Form is needed if we are to give medicine to your child during the school day.
